By initialing below, I acknowledge and agree to the following terms and conditions of employment with MycoCorp Research Facility:

1. ASSUMPTION OF RISK: I understand that employment at MycoCorp involves inherent risks including but not limited to: exposure to experimental fungal spores, biological containment breaches, chemical exposure, radiation exposure, dimensional anomalies, sentient organism encounters, facility structural failures, mysterious disappearances, unwanted biological mutations, spontaneous combustion, temporal displacement, and/or complete molecular dissolution.

2. RELEASE OF LIABILITY: MycoCorp, its parent companies, subsidiaries, affiliates, executives, board members, scientists, janitors, and shareholders SHALL NOT BE HELD LIABLE for any injury, dismemberment, death, undeath, parasitic infection, mind control, personality alteration, species transformation, loss of consciousness, loss of sentience, corporeal degradation, soul fragmentation, or complete erasure from the space-time continuum that may occur during the course of employment.

3. MEDICAL CONSENT: I hereby authorize MycoCorp medical staff to perform any emergency procedures deemed necessary, including but not limited to: experimental antidote administration, emergency amputation, spore extraction, memory suppression, and/or complete biological reconstitution.

4. CONFIDENTIALITY: I agree to maintain absolute secrecy regarding all MycoCorp operations, experiments, incidents, accidents, cover-ups, and any knowledge of Projects Sporefall, Mycelia Network, or Sentience Initiative under penalty of immediate termination and/or voluntary memory erasure.

5. MANDATORY ARBITRATION: I agree that ANY AND ALL disputes, claims, or legal proceedings arising from my employment, including but not limited to wrongful death, negligence, fraud, crimes against nature, violations of the Geneva Convention, or interdimensional treaty violations MUST be resolved exclusively through binding arbitration with a MycoCorp-appointed mediator. All arbitration proceedings SHALL BE HELD at the MycoCorp Main Office located in District 1, regardless of my current dimension of residence or state of corporeal existence.

6. NEXT OF KIN: I acknowledge that MycoCorp is not responsible for notifying my next of kin in the event of my disappearance, transformation, or transition to an alternate plane of existence. Any notifications will be made at MycoCorp's sole discretion and convenience.

7. PROPERTY RIGHTS: Any mutations, abilities, or biological changes that occur during my employment are the sole intellectual property of MycoCorp. I waive all rights to my post-transformation self.

8. SURVIVAL CLAUSE: This agreement remains in effect in perpetuity, surviving my employment termination, death, reanimation, or evolution into a higher consciousness.
